From the moment someone is referred to Hope Village through long-term residency, every step is designed to support stability, health, and community connection.
Phase 1
Referral & Screening
- Referral received through coordinated entry system
- Initial eligibility screening completed by intake staff
- VI-SPDAT assessment administered
- Applicant added to prioritized housing list
Phase 2
Intake & Move-In
- Unit match identified and offer extended
- Lease signing and document review with housing navigator
- Move-in day with on-site staff support
- Introduction to community, services, and neighbors
Phase 3
Stabilization
- Regular check-ins with assigned case manager
- Connection to healthcare providers and medical management
- Benefits enrollment and financial stability support
- Participation in community programming and activities
Phase 4
Long-Term Residency
- Ongoing wraparound services tailored to resident needs
- Annual lease renewal and housing plan review
- Peer support and community leadership opportunities
- Continued connection to health, wellness, and social services
Hope Village is a permanent home — not a transitional program. Residents are not expected to "graduate" out of housing. Our goal is long-term stability and community belonging for every person we serve.
How to apply
Entry Process
We provide safe, stable housing for single adults who are experiencing homelessness and have significant health needs. See if our community is the right fit for you.
Check the Basics
We prioritize individuals who are:
- Unhoused: Currently living in a shelter, on the street, or in a place not meant for human habitation.
- Medically Fragile: Have a serious, long-term health condition.
- Single Adult: 18 years or older and applying as an individual.
- Community Ready: A good neighbor willing to work with our support staff.
Start the Conversation
Reach out to our intake team or a partner agency. A trained staff member will meet with you to listen to your story and understand your needs. This first meeting is a simple screening to confirm your situation.
Tell Your Story
You'll complete a standard assessment called the VI-SPDAT with a staff member. This is not a test — it's a tool that helps us understand your unique challenges, especially concerning your health and housing history. This ensures we prioritize those with the most urgent needs.
Get On the List
Based on your assessment score, you will be added to our community's prioritized list for housing. When a unit at Hope Village becomes available that matches your needs, our team will contact you to begin the final steps.
Welcome Home
Once you are matched, you'll work with a housing navigator to get your documents in order and move into your new home. You will also be connected with our on-site support services to help you stay housed and healthy.

Code of Conduct
At Hope Village, we are committed to creating a safe, supportive, and respectful community for all residents. The following code of conduct, based on the principles of HOPE, serves as a guide for how we interact with one another and care for our shared environment.
Honor and Respect
- Treat fellow residents, staff, and visitors with kindness, dignity, and respect.
- Honor the diversity of our community by embracing differences in culture, background, and beliefs.
- Respect the privacy and personal space of others, including their living areas and belongings.
Ownership of Actions
- Take responsibility for your actions and their impact on the community.
- Follow community guidelines, including those related to cleanliness, noise levels, and shared spaces.
- Report maintenance issues, safety concerns, or conflicts to staff promptly and constructively.
Participation in Community
- Engage in community activities, meetings, and events to build connections and foster a sense of belonging.
- Contribute to the upkeep of shared spaces by cleaning up after yourself and encouraging others to do the same.
- Support fellow residents by offering encouragement, understanding, and a helping hand when needed.
Excellence in Care
- Strive to maintain a clean, safe, and welcoming living environment for yourself and others.
- Take care of your personal health and well-being by utilizing the resources and services available at Hope Village.
- Uphold the values of Hope Village by being a positive role model and promoting a culture of compassion and accountability.
