If you are a case manager, service provider, healthcare worker, or community member working with an individual who may be eligible for permanent supportive housing, we want to hear from you.
To make a referral or request contact:
- Submit a referral through the coordinated entry system in Champaign-Urbana
- Reach out directly via our contact form for guidance on next steps

All referrals are reviewed in accordance with coordinated entry protocols. Individuals do not need to self-refer — advocates, case managers, and service providers are encouraged to reach out on their behalf.
