Hope Village is a permanent supportive housing community coming to Champaign-Urbana in Fall 2026, purpose-built to serve individuals experiencing chronic homelessness, specifically for those with complex medical needs.
What is Hope Village?
Developed to address one of the most persistent challenges in Champaign-Urbana — the lack of stable, long-term housing for individuals experiencing chronic and medically complex homelessness — Hope Village provides permanent supportive housing (PSH) alongside wraparound services designed to help residents stabilize, heal, and thrive.
Our mission is straightforward: to ensure that every person, regardless of their background or health status, has access to safe, dignified, and sustained housing.
Hope Village is not a shelter or a transitional program. It is a permanent home — one built on the belief that housing is a foundation for everything else.

What is medical fragility?
Medical fragility refers to individuals whose health is significantly worsened by homelessness — and whose conditions cannot meaningfully improve without stable housing as a foundation. This includes people with frequent ER visits, complex treatment needs, or life-limiting illness.

How residents come to Hope Village
Hope Village operates within Champaign-Urbana's coordinated entry system, ensuring that individuals are matched to housing in an equitable, needs-based way.
Individuals connect with the local homeless services system through outreach programs or emergency shelter. This is where their needs are first assessed.
Case managers work with individuals to assess their housing barriers, medical needs, and eligibility for permanent supportive housing through the coordinated entry process.
Eligible individuals are referred to Hope Village through the coordinated entry system. Priority is given to those with the highest needs and longest histories of homelessness.
Once matched, residents go through a structured intake process and transition into their new home at Hope Village with on-site support from day one.
Please note: Not everyone who is referred will qualify for placement at Hope Village. However, our intent — and our commitment — is to support as many individuals as possible and to connect those who do not qualify with other appropriate resources within the continuum of care.
